Thanks, John.

Even then, the form leaves a lot to be desired -- it looks like the form
for requesting a new NetID (no doubt because it actually is). I don't have
3 choices for the NetID -- it's an existing one. And I don't want my
project NetIDs listed in people search, etc.

If anyone from IT Services would like to comment on just what we need to
fill out in this form if we're just trying to prevent expiration, that
would be helpful. A completely separate form just for renewals would have
been nice, but I suppose we're stuck shoehorning those requests into this.
