3月19日 ( 星期六 ) 在 Chinese Community Center ( Madison Heights, MI ) ，芝
加哥总领馆办理护照，旅行证，底特律中国人协会将代办中国签证
为服务广大侨胞，中国芝加哥总领事馆将于2016年3月19日 ( 星期六 )赴底特律，在
Chinese Community Center ( 美华中心 ) ( Madison Heights, MI ) 从上午9时到下
午2时受理在美有合法居留人士的中国护照、旅行证、健在证明和香港特区护照申请，
底特律中国人协会将协助领事馆办好这次活动，并将于3月18日 ( 星期五 )晚
6:30-8:30在坎通中文学校, 3月19日 ( 星期六 )在 Chinese Community Center ( 美
华中心 ) ( Madison Heights, MI ) 代办中国签证。现将相关事项通知如下：
一， 办证安排：
1. 时间： 2016年3月19日 (周六) 上午9:00时 – 下午2:00时.
这次办理健在证明无需预约，随到随办，但办理护照需事先预约，请使用以下链接：

二， 办证须知：
1，所有中国护照、旅行证、健在证明和香港特区护照申请的申请人请登陆芝加哥中国
总领事馆网站：http://www.chinaconsulatechicago.org/chn/qzhz/hz/ 请务必按照要
求准备好申请材料。
2，填妥并签名的《中华人民共和国护照/旅行证/海员证/回国证明申请表》一份，此表
下载网址为：http://www.chinaconsulatechicago.org/chn/qzhz/hz/
3，请提前准备好贴好邮票的美国邮局(US Postal Services) 的EXPRESS MAIL信封。收
件人和发件人均请填写申请人自己的姓名和在美国的境内地址。请自行将Customer
Copy取下来，以便日后查询。
三， 办证收费标准：
1） 芝加哥中国总领事馆：换发护照：$35 + $5（回邮服务费）；补发护照：
$35 + $5（回邮服务费）。几份换发护照一起申请只收一次回邮服务费，支付一张现金
支票。现金支票（Money order or Cashier’s Check），金额包括办证费用和回邮服
务费，支票抬头请写：Chinese Consulate in Chicago.
2） 底特律中国人协会将按下述标准收取服务费：底特律中国人协会会员每份
申请收费 $20，非会员每份申请收费 $30。健在证明收服务费 $10。非会员可当时参加
底特律中国人协会（一次性收费$20/family），并享受会员的价格。支票抬头请写：
CAGD (Chinese Association of Greater Detroit)。
3） 申请人需要付两张支票，一张为现金支票（Money order or Casher’s
Check）写给芝加哥中国总领事馆（Chinese Consulate in Chicago），另一张支票写
给底特律中国人协会(CAGD)。

2016年3月18日 ( 星期五 ) 晚6:30到 8:30在坎通中文学校办理中国签证 (坎通中文学
校地址: Plymouth Christian Academy, 43065 Joy Road, Canton, MI 48170); 不用
预约。
2016年3月19日 上午9点 到 下午2点在Chinese Community Center ( 美华中心 ) (
Madison Heights, MI ) 办理中国签证。办理签证需事先预约，

办中国签证的费用和底特律中国人协会收取的服务费均可用现金，个人支票，或现金支
票（Money order or Cashier’s Check），支票抬头请写给底特律中国人协会(CAGD).
加拿大护照办签证领馆收费 $90。美国护照办签证领馆收费 $140。
** 中国人协会一贯致力于高效地为大家服务，签证一周取件，将于3月26日在1300
Derby Road, Birmingham, MI 48009发还。

(c) Visa Fee: Effective January 15, 2010, the Chinese visa fees for U.S.
citizens will be adjusted on reciprocal basis to be USD140 for individual
visa.
(d) Service Fee:
$40 for each application for a ten-year multiple entry visa for Non-members
of CAGD.
Member of CAGD will have a $10 discount.
Payment can be made by cash, Personal check, and Money Order or Cashier’s
check, payable to CAGD.
